TO PAY BY CREDIT CARD: Complete the form and confirm all data entered. Then go to PayPal for online payment by credit card. To avoid a late fee, the online payment must be received by July 3, 2008 at 5:00 p.m. EST. The system will automatically charge $225.00 after 5:00 p.m. EST.

Seminar materials will be mailed to persons registering by July 15, 2008.
